Abused women victims of 'fundamental failures'

with VIDEO and ANALYSIS

The Western Australian Government has for the first time conceded there were "fundamental failures" in the way the police and government agencies handled calls for help from a woman who was in mortal danger from her violent husband.

Andrea Pickett was murdered by her estranged husband, Kenneth Pickett, in January 2009, after he had made repeated threats to kill her.

Ms Pickett’s family fought for two years for an inquest to take place in order to highlight the systemic failures which allowed her husband to carry out his threats.
